Montecchio di Giano (PG):
Church of S. Rocco

The little brown sign in the photo above states, on what grounds I can't tell you, that this one-room church on the outskirts of Montecchio dates to the 14c. The visitor can get that on our own: St. Rocco was, with St. Sebastian, the great protector against the plague, and the plague struck Europe in the 1340s.

The actual architecture, in fact, to me looks earlier: 12c or 13c; churches can and do change name.
